Denver-based Orthopedic Centers of Colorado will deliver real-time patient guidance through Clarify Health Solutions technology.

1. OCC has already served more than 100 patients through the Clarify Care Journey Platform.
2. The Clarify Care Journey Platform is comprised of Care Connect and Care Pilot. It was developed to help providers guide patients through the care journey, improve patient outcomes and streamline physician workflow.
3. Clarify Care Connect provides step-by-step directions for patients and notifies care teams when an action or intervention is needed.
4. Clarify Care Pilot is a mobile- and browser-based platform connecting patients with the care team through two-way messaging, smart device connectivity and the ability to report progress.
