1st orthopedic episode care management platform linked to AAOE Data Warehouse — 5 key insights

Signature's CareMOSAIC is the first episode care management platform to connect to the American Association of Orthopaedic Executives' Data Warehouse.

Here are five things to know:

1. CareMOSAIC's web-based case management platform has reduced post-acute care costs by an average of 42 percent and achieved reductions across a range of adverse outcomes, including a 24 percent reduction in 30-day hospital readmission rates.

2. CareMOSAIC allows financial and clinical elements of care to be integrated into real-time analytics, streamlining case management workflow.

3. Case managers already report on patient-reported outcomes in CareMOSAIC, and now those results can be uploaded directly to AAOE's Data Warehouse.

4. The AAOE Data Warehouse provides orthopedic practices with direct access to national benchmarking data for value-based care decision-making. The Data Warehouse is a CMS-approved 2018 Qualified Clinical Data Registry intended to help providers fulfill their MIPS reporting requirements.

5. Signature Care Management currently manages the largest orthopedic bundled payment program in the U.S., overseeing 55 orthopedic practices in 27 states and managing 45,000 Medicare surgical episodes annually.
