Atlanta-based Peachtree Orthopedics faced a data breach on April 20 that impacted patient information, including names, addresses, birth dates, driver's license numbers, Social Security numbers, medical treatment/diagnosis information, treatment costs, financial account information and health insurance claims/provider information.
