Here are five things to know:
1. Great Lakes Organized Systems of Care, a physician-led organization focused on patient experience and outcome improvement, is partnering with the ACO.
2. Great Lakes Physicians ACO was formed to focus on care coordination, population health and preventative care.
3. The ACO is focusing on commercially-insured patients and offers a network of physicians for primary care.
4. The ACO is one way for physicians to stay independent and will operate under a value-based payment structure. The physicians are rewarded for positive outcomes instead of services delivered.
5. There are 15 practices that jointed Great Lakes Physicians ACO and Aledade is continuing to recruit “high-quality practices” for the ACO.
