The program launched Jan. 1 and includes physicians affiliated with Primary Physician Partners and South Metro Primary Care — Denver-area groups that are part of Physician Health Partners.
As part of the CAC initiative, physicians will coordinate patient care with the help of registered nurses and social workers, who will serve as care coordinators. Cigna will provide patient-specific data to the providers.
