Under the medical home system, physicians have been using EMRs to track their patients’ blood pressure, blood sugar, exercise levels and overall visits, the report said.
Roughly 13 percent of all physicians and pediatricians work in practices set up as patient-centered medical homes, according to the report, and they are communicating with specialists, nutritionist and others through EMRs to formulate healthcare plans as well.
