The payments include “no-show” fees of $30-$50 for missed appointments, charges to fill out health forms and annual administrative fees of $35-$120 to be a patient of the practice.

The Medical Group Management Association, WellPoint and the Arizona chapter of the American Academy of Pediatrics reported more physicians are asking for such fees.
