The most recent data breach was when a staff member at the Lexington (Ky.) VA Medical Center took home patient files, slides, images and data on his laptop without authorization. The data held approximately 1,900 veterans‘ personal information, including names, the last four digits of Social Security numbers, dates of birth and medical diagnoses.
