Here are some tips for coding physical and occupational therapy service categories in CPT:

1. Evaluations can include assessments and documentation of the patient’s history, function level, systems review, specific tests, diagnosis and prognosis. Unique evaluation and re-evaluation codes for physical therapists include CPT codes 97001 and 97002. Codes for occupational therapists include CPT codes 97003 and 97004.

2. CPT defines supervised modalities as no timed services under CPT codes 97010-97028. Other modalities defined as “constant attendance” services requiring one-on-one contact with the provider can be coded with CPT codes 97032-97039 and are billable in 15-minute increments.

3. Therapeutic procedures are timed and require direct patient contact. The CPT codes are 97110-97546.

4. Active wound care management includes services that promote healing through removing devitalized and necrotic tissue from the patient’s body. They can be coded as CPT codes 97597-97606.

5. Tests and measurements as a component of evaluation and re-evaluation. They can be coded using CPT codes 97750-97755.

6. Orthotic and prosthetic management is reported as a unique service using CPT codes 97760-97762.
