On Oct. 15, TSC filed a notice of the breach with the Attorney General of Texas, according to an Oct. 17 press release.
An internal investigation that ended Sept. 5 determined that patient names, Social Security numbers and other sensitive information was impacted. The incident affected 8,048 individuals.
