The Jan. 30 notice warns that an unauthorized party had access to SCOSM’s system on or around Nov. 24, according to a Feb. 27 article from law firm Console and Associations published by JD Supra.

Breached patient information includes patient names, Social Security numbers, driver’s license numbers, billing information, and health insurance and medical information.

SCOSM detected suspicious activity on Nov. 24, and by Dec. 9 determined files and folders were compromised. The practice then conducted an internal investigation that concluded Jan. 29.
