Sports Medicine and Orthopedics, a now-closed orthopedic practice in East Providence, R.I., reported a data security incident.

The practice, which has been closed for about 10 months, experienced a ransomware attack involving patient information stored on an electronic medical record system, according to a Nov. 28 notice from Sports Medicine and Orthopedics.

On Oct. 10, Sports Medicine and Orthopedics found that the server had been encrypted by ransomware. The record system was not in active use at the time of the incident.

Patient information that may have been accessed includes names and addresses. The practice said there is no evidence that any data has been misused.

The breach affected 4,000 individuals, according to the HHS Office for Civil Rights.
