4. A Transitional Care Unit’s biggest expense is in the first couple days with assessments, requirements of MDS (a charting system CMS requires of skilled nursing facilities) and a Medicare payment determined by therapy minutes.
5. Post acute care providers need to be at the table as bundles are designed, so all parties in the continuum understand each other’s worlds, incentives and limitations.
