Arizona orthopedic practice suffers data breach

Integrated Orthopedics of Arizona sent out a notice of a data event that may affect personal information. 

On or around April 7, the practice became aware of suspicious activity in its email tenant. An investigation found that some emails may have been copied without authorization, according to the Aug. 11 notice. 

Information that may have been impacted includes names, addresses, dates of birth, medical record numbers, patient ID or account numbers, Medicare or Medicaid numbers, health insurance ID numbers, health insurance group numbers, medical diagnosis information and medical treatment information.

Integrated Orthopedics of Arizona has a clinic location in Scottsdale and a surgery center in Phoenix.
