The practice was alerted to the unauthorized access of a corporate email account Aug. 6, according to a notice on the company’s website. On Oct. 28, the practice learned that files may have been removed from the impacted email account by an unauthorized third-party actor between June 20 and Aug. 6.
The information that may have been obtained includes names, dates of birth, Social Security numbers, government identification numbers, credit card information, medical information and insurance information.
The practice said it has no evidence that any personal information has been misused.
The incident was submitted to the U.S. Department of Health and Human Services Office for Civil Rights Breach Portal Nov. 22.
