• Transition to a direct-pay practice — 7.6 percent

• Join an accountable care organization — 4.4 percent
• Transition to a concierge practice — 4.1 percent
• Leave my practice to become hospital employed — 4 percent
• Become a patient-centered medical home — 3.9 percent
• Merge with other practices — 3.8 percent
• Go into solo practice — 3.8 percent
• Other — 12.9 percent
