Here’s what you need to know.
1. In an effort beginning in 2010, Medicare and hospitals around the country have seen “substantial” progress in keeping patients safe and reducing readmissions.
2. Medicare has set several goals for hospitals to meet. One such goal is: lowering preventable medical problems in hospitals from 121 per 1,000 patients discharged to 97 per 1,000 by 2019.
3. To facilitate these goals, Medicare is awarding $347 million to hospitals and health organizations.
