Here are four things to know:
1. The new office is expected to open around Sept. 1, 2018.
2. It will occupy a 12,000-square-foot space.
3. Mayfield will lease the clinical office and space for physical therapy from Hines Property Management.
4. A Green Township, Ohio-based office is also being planned, according to Cincinnati Business Courier.
