3. In the partnership, Humana will provide practices with data and analytics to reduce readmissions and complications.
4. In North Carolina, Humana will work with:

• OrthoCarolina in Charlotte
• Wake Forest University Health Sciences in Winston-Salem
5. The following Virginia-based practices will participate:
• Appalachian Orthopaedic Associates
• Hampton Roads Orthopaedics & Sports Medicine in Newport News
• Jordan-Young Institute in Virginia Beach
6. Humana has already implemented its value model with 12 orthopedic groups in Ohio and Tennessee and eight groups in Indiana and Kentucky.
