2. Hospitals and ambulatory surgery centers seeking HFAP Joint Replacement Certification must:
- Hold current accreditation with deemed status or Medicare certification by the state
- Meet state licensure requirements
- Maintain compliance with all applicable HFAP certification standards
- Define the population served by the joint replacement program within its protocols
- Provide a minimum of 12 consecutive months of applicable performance measures or one month of data prior to applying for initial certification
3. HFAP is CMS-authorized to survey all hospitals for compliance with the Medicare Conditions of Participation and Conditions for Coverage.
