1. The HFAP Joint Replacement Certification program has options for outpatient and inpatient orthopedic settings.

2. Hospitals and ambulatory surgery centers seeking HFAP Joint Replacement Certification must:

  • Hold current accreditation with deemed status or Medicare certification by the state
  • Meet state licensure requirements
  • Maintain compliance with all applicable HFAP certification standards
  • Define the population served by the joint replacement program within its protocols
  • Provide a minimum of 12 consecutive months of applicable performance measures or one month of data prior to applying for initial certification

3. HFAP is CMS-authorized to survey all hospitals for compliance with the Medicare Conditions of Participation and Conditions for Coverage.
