The top six challenges of embedding a case manager are:

 

1. Lack of CM peer support — 15 percent
2. Staff buy-in — 15 percent
3. Communication — 12.5 percent
4. Reimbursement — 12.5 percent
5. Patient engagement — 10 percent
6. Data sharing — 10 percent
