5 Steps for Properly Terminating Patient Relationship

The five steps are:

 

1. Give appropriate written notice.
2. Give a brief explanation of the reasons for termination of the relationship.
3. Agree to continue to provide treatment and access to services for a reasonable period of time to allow the patient to secure care from another person.
4. Provide resources or recommendations to help the patient locate another physician of like specialty.
5. Offer to transfer the patient’s records to a designated person.
