According to Dr. Stiegler, an “adverse event” includes any unexpected and unfavorable outcome, regardless of whether an error occurred. Five suggestions on elements to include in disclosure conversations are:
1. Discuss the facts, without speculating about causality.
2. Offer a timeline for the analysis process so patients know what to expect.
3. It may be appropriate to disclose whether a medical error occurred and what steps will be taken to ensure it doesn’t happen in the future.
4. Expressing sympathy whether or not a medical error has occurred may be helpful.
