CMS has published the list of hospitals that will take part in its Comprehensive Care for Joint Replacement Expanded Model, known as CJR-X. As of Sept. 30, 2026, 2,217 acute care hospitals meet the participant definition. CMS said it plans to update the list on a regular cadence to reflect newly opened hospitals, mergers and closures.
CJR-X was finalized in the fiscal 2027 IPPS final rule and will be mandatory nationwide beginning Jan. 1, 2028. The model covers Original Medicare patients undergoing hip, knee and ankle replacements in inpatient and outpatient hospital settings. Participating hospitals will be responsible for patients’ care from the time of the procedure through the first 90 days of recovery.
After each performance year, actual episode spending will be compared with the hospital’s target price. Depending on quality and spending performance, the hospital could receive an additional Medicare payment or be required to repay part of the spending.
The model expands the original CJR program. That program ended Dec. 31, 2024, after being tested in 34 metropolitan statistical areas, and its most recent evaluation estimated $112.7 million in net savings while maintaining quality of care.
Hospitals participating in the Transforming Episode Accountability Model are exempt, as are hospitals in Maryland and those not paid under both the inpatient and outpatient prospective payment systems . Maryland is one of two states with no hospitals on the list; the other is Rhode Island.
