CMS is expanding its Advancing Chronic Care with Effective, Scalable Solutions Model to provide ongoing support for certain specified conditions within its chronic musculoskeletal pain track beyond the initial 12-month care period, beginning April 1, 2027.
The model provides technology-supported care options for people with Original Medicare. Its musculoskeletal track includes chronic musculoskeletal pain and can incorporate virtual care, remote monitoring and other technology-enabled services, according to a Sept. 15 news release.
Under ACCESS, participating organizations receive recurring payments for managing qualifying conditions, with full payment tied to achieving measurable health outcomes. The model is designed to complement traditional care, allowing referring clinicians to send patients to ACCESS organizations and receive updates on their progress.
The 10-year model began in July and has 160 organizations participating at launch. Three out of four people with Medicare qualify for at least one ACCESS track.
The April 1 expansion also adds tracks for heart failure, chronic obstructive pulmonary disease, substance use disorder and tobacco cessation. ACCESS is available to people with Original Medicare; Medicare Advantage enrollees are not eligible.
