Digital care management platform reduces total joint readmissions for California hospital

Patient engagement platform Force Therapeutics said its episode-based digital care management platform helped a hospital reduce its 90-day total joint readmissions more than 26 percent.

The finding comes from a one-year retrospective analysis.of readmissions at Community Hospital of the Monterey (Calif.) Peninsula.

Force Therapeutics said in a Jan. 31 press release that the study examined readmission rates before and after implementation of its platform, which educates patients on identifying infections and other recovery complications and enables an individualized care plan.

Highly engaged patients were far less likely to be readmitted, Force said, especially for total hip procedures.
