HHS was notified Feb. 3 about the theft, which potentially compromised 530 patients’ data. No business associate was present when the theft occurred.
Arizona Pain and Spine Institute has locations in Mesa, East Mesa and Queen Creek. The practice’s co-founders are Asim Khan, MD, and Daniel Ryklin, MD.
Dr. Khan specializes in physical medicine, rehabilitation, interventional pain medicine and sports medicine. Dr. Ryklin specializes in the management of acute and chronic spinal disorders, electrodiagnostic medicine and musculoskeletal medicine.

From the start of the year through Feb. 13, HHS received reports of data breaches from 41 healthcare entities.
Most of the reports involved hacking incidents, IT incidents, unauthorized access or unauthorized disclosure, according to JD Supra. Cumulatively, the breaches potentially affected the data of over 1 million patients.
