Fluoroscopic Navigation for Pedicle Screw Placement: Does 3D Make a Difference?

A systematic review of studies on the accuracy of intraoperative image guidance systems for pedicle screw placement was recently published in the Journal of Neurosurgery: Spine.

The researchers examined 30 studies that analyzed the use of conventional fluoroscopy, two-dimensional fluoroscopic navigation and three-dimensional fluoroscopic navigation. There were 1,973 patients involved in the studies and 9,310 pedicle screw insertions.

 

The researchers found:

 

•    68.1 percent accuracy with the conventional fluoroscopy
•    84.3 percent accuracy with 2D fluoroscopic navigation
•    95.5 percent accuracy with 3D fluoroscopic navigation

 

“Three-dimensional fluoroscopic image guidance systems demonstrated significantly higher pedicle screw placement accuracy than conventional fluoroscopy or 2D fluoroscopic image guidance methods,” the researchers concluded.
