Of the 30 largest cities in the U.S., San Jose, Calif., is the most expensive place to receive cervical spine surgery, according to Healthcare Blue Book. However, the physician services fees are highest in Charlotte, N.C., for cervical spinal fusions, hitting $9,903.

Physician services fees were typically between $3,000 and $4,000 in the largest cities. Here is the breakdown for average physician services fees for the 30 largest U.S. cities, based on the 2010 census.
