Hybrid vs. open adult spinal deformity surgery: Which is best? 8 things to know

A study published in the Journal of Neurosurgery: Spine examines two minimally invasive surgery strategies for treating adult spinal deformity.

The study authors conducted a multicenter study examining data from 105 patients with adult spinal deformity who were treated with minimally invasive techniques. The patients either underwent the hybrid surgical approach with minimally invasive lateral interbody fusion with open posterior instrumented fusion or the circumferential minimally invasive surgical approach.

 

The patients were 60.7 years old on average in the hybrid group and 61 years old in the circumferential MIS group. The researchers found:

 

1. There were an average of 3.6 interbody fusions performed in the hybrid group. In the circumferential MIS group, there were four interbody fusions performed on average.

 

2. The posterior fusion involved around 6.9 levels in the hybrid group, compared to 5.1 levels in the circumferential MIS group.

 

3. The average Oswestry Disability Index score improved by 30.6 for the hybrid group and 25.7 for the circumferential MIS group.

 

4. The average visual analogue scale for back/leg pain improved by 2.4/2.5 for the hybrid group. For the circumferential group, the scores improved by 3.8/4.2.

 

5. There weren’t any significant differences for ODI or VAS scores between the two groups.

 

6. For the hybrid group:

 

Lumbar coronal Cobb angle decreased by 13.5 degrees
Lumbar lordosis increased by 8.2 degrees
Sagittal vertical axis decreased by 2.2 mm
LL-pelvic incidence mismatch decreased by 8.6 degrees

 

7. For the circumferential MIS group:

 

Lumbar coronal Cobb angle decreased by 10.3 degrees
Lumbar lordosis increased by 3 degrees
Sagittal vertical axis decreased by 2.1 mm
LL-pelvic incidence mismatch decreased by 2.2 degrees.

 

8. There weren’t any differences in the radiographic parameters between the two groups, but the complication rate was higher for the hybrid group at 55 percent than the circumferential MIS group at 33 percent.
