Here are four takeaways:

 

1. Medical culture must become more reflective and team-oriented to encourage open discussion, routine monitoring and feedback concerning diagnostic performance.

 

2. Staff interventions need to target all diagnostic errors, regardless of their potential to injure patients.

 

3. A national coalition of professional societies needs to translate the National Academy of Medicine's recommendations concerning preventing diagnostic errors into action. Such recommendations include increased communication and patient engagement.

 

4. The coalition should also include organizations that represent primary care physicians, teachers and researchers who realize the patient safety risks present in 15-minute office visits.
